Students w/ Medical Conditions

Does your child have:

Allergies, Asthma, Diabetes, Seizures, CP, MS, CA… any chronic health condition????

 
Help us to HELP your child! 
 

We must have an HCS action plan on file from your child's doctor!

I can provide you with the form… you provide it to your doctor… and return it to me.

 

If your child has a chronic condition we will happily work with you in following doctors orders.

We have special forms that MUST be completed by your physician and returned to the clinic.

These forms tell us how your doctor would like us to care for your child and are in addition to the

medication permission forms already mentioned.

 

MEDICATIONS ARE TO STAY IN THE CLINIC AT ALL TIMES.

 

We are always happy to work with your doctor and will provide them with any information you wish.
